Responsibility

  • the post holder will be involved in many aspects of medicines management and be supported and developed in this role by the PCN clinical pharmacy team
  • undertake any other additional duties appropriate to the post as requested by the PCN Clinical Directors or network practices
  • work with the pharmacy team and Clinicians to deliver QIPP and other local commissioned enhanced services to improve medicines optimisation
  • to attend and participate in practice clinical meetings and other multi-disciplinary meetings where necessary
  • ensure that all activities are monitored and evaluated
  • attend any training courses and supervision sessions as required
  • undertake any other duties as may reasonably be required from time to time
  • develop positive and professional working relationships with colleagues to ensure excellent patient care

Responsibility

  • use data to proactively identify and work with a cohort of patients to deliver personalised care
  • help people to manage their needs, answering their queries and supporting them to make appointments

  • support people to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing
  • support the coordination and delivery of MDTs within PCNs

  • work with Primary Care professionals within the PCN to identify and manage a caseload of patients, be involved in co-ordination of care and where required and as appropriate, refer people back to other health professionals within the PCN
