Responsibility
- assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits
- support the implementation of national prescribing policies and guidance within the GP practice and care homes
- supporting quality improvement measures and contributing to all appropriate elements of the delivery of the Directed Enhanced Service
- carry out medicine's optimisation tasks including effective medicine administration , supporting medication reviews and medicines reconciliation
- support the role of the Clinical Pharmacist at GP practice level on the delivery of the Clinical Pharmacy in General Practice model
- where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively
- support the Clinical Pharmacist in Structured Medication Reviews i.e., organise necessary monitoring tests prior to SMR Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients
- carry out housekeeping whilst in patient records e.g., dose optimisation and medicines synchronisation
