  • bringing safeguarding concerns to the attention of a relevant GP and acting in accordance
  • exploring and assisting patients with personal health budgets where appropriate
  • working with GPs and other primary care professionals within Liverpool First PCN to identify and manage a caseload of patients
  • supporting patients utilise decision aids in preparation for shared decision-making conversation
