Responsibility
- they will ideally have experience within chronic disease management, COPD and Asthma
- · Demonstrate an ability to undertake baseline physical observations and record results correctly
- experience in diabetes would be advantageous
- in addition be able to identify anomalies that should be referred to the GPN / ANP or GP Medical staff
- demonstrate evidenced based knowledge in Mental problems/ Substance Misuse, which may affect physical health
- · Articulate an understanding of MDT in and staff roles/ Mental Health, community Nursing 0-19 services etc
- we are looking for a team player who is able to assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
