Responsibility
to lead on communication with primary care practitioners, local authority partners, health staff, the neighbourhoods and patient / carer groups
support and assist the 8a leads with any workforce matters around recruitment of staff to the Virtual Ward
to work with the Virtual Ward clinical leads to co-ordinate and agree priority areas and metrics / measures, whilst ensuring that the personalisation agenda is adhered to
ensure that transition pathways between the hospital the virtual ward and social care services are fully developed and safe, to ensure that frail and vulnerable older adults get the support they need during their time with the Virtual Ward and on discharge
act as a professional lead for the project - providing clinical expertise and oversight in the development of the project and enlisting specialist clinical expertise and support as required
act as overall project manager, co-ordinating and monitoring the completion of all pre and post go live plans and liaising with corporate service leads ensuring that all tasks are completed within timescales and allocating tasks as appropriate and ultimately making decisions in the best interest of the project and clinical service
Requirements
significant relevant and current clinical experience working within adult health and or social care service
experience of working in partnership with clinicians and wider partner organisations
significant experience of forging and maintaining strong working relationships at all grades
senior hr advisor
Requirements
proven experience in a HR role that included undertaking an employment relations component with minimal supervision
experience of advising managers on the application of employment policies
up to date knowledge of employment law and good employment practice
