Nurse - Development Role London
The successful candidate will be able to provide and deliver safe patient care in the community by care coordinating a caseload of patients. The post holders main focus will be to coordinate all aspects of the patients care from admission to recovery and transfer/discharge. The post holder will be able to form therapeutic, meaningful relationships with patients and ensure they are actively involved in both the planning and delivery of their care.
