- undertaking recruitment work aimed as specific parts of the community where there is an under-representation of Foster Carers, ensuring that the approach and materials are appropriate to the community being targeted
- recruitment of Foster Carers, as well as the training and assessment of prospective Foster Carers and Connected People in accordance with legislation, guidance, standards and good practice appropriately in order to meet the needs of Looked After Children and young people requiring substitute care
- undertake initial interviews with prospective Foster Carers as required, providing verbal and written information and making professional and informed judgments on the suitability of applicants to be approved as Foster Carers in accordance to standards and safeguarding practice

- the role holder will carry out social care assessments; support plans/care plans and reviews within the Integrated Health and Care Pathway with individuals and families who need specific support
- the main purpose of this job is to work as a care management social worker to assess the needs of vulnerable adults and their carers, and to support them to arrange services to meet the needs and outcomes identified
- work with Community Development Staff to help people access community based services
- the role will be responsible for the management, oversight and development of Child Protection and Court work to ensure that children have the right plan at the right time, and that all plans progress in a timely manner and intervention is effective support children within their families and prevent the need for the plan to escalate
- we have created a dedicated Child in Need Service, this has allowed us to create a dedicated service to work with children and families when things escalated to Child Protection in the Support and Protect Service
- this role is a mixture of home and hospital - Will be required to attend the hospital for up to 3 days per week to support with duty and visit the wards to undertake assessments and attend family meetings

- the role requires social workers to travel to see children wherever they are placed, and this could be nationwide. There is not an expectation to be office based but to participate in some in person events and meetings
- they will manage a caseload of individuals and carers where the supportplan/care plan has been put in place and may need specific support, closemonitoring, frequent review, or where interventions to minimise and manage risk ormaintain protection plans may be required
- risk management and safeguarding will also be key parts of this role , together with case management support for those adult requiring frequent specialist intervention to meet their needs
- ensure that risk management and adult safeguarding procedures are adhered to in social work practice
- to ensure that appropriate care and resources are made available to service – users with substance misuse needs, and to regularly review the care plan, and adjust services where appropriate
- the team are on duty two days a month. The social worker is expected to be in the office for duty days
- to be considered, you must have a Social Work qualification as well as at least 2 years post qualified experience working as a Qualified Social Worker within a similar team
- ensure service users and their carers receive timely and proportionate care management assessments of their needs that identify the outcomes they wish to achieve
- this isn’t necessarily a specific caseload number as we all know that in our practice numbers don’t always reflect the complexity of the children and families you are working with
- to implement Community Care Legislation Fair Access to Care Services criteria, the Mental Capacity Act, and Mental Health Act to meet service – users needs
