Responsibility

  • undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients
  • carry out medicines optimisation tasks including effective medicine administration , supporting medication reviews and medicines reconciliation
  • reviewing new patient medications, hospital discharge and/or clinic letters ensuring accurate information is recorded on the patient record
  • supporting the Clinical Pharmacist in Structured Medication Reviews i.e
  • organise necessary monitoring tests prior to SMR
  • managing shared care protocols and liaise with the Clinical Pharmacist for more complex patients
  • synchronising quantities on prescriptions and align medications to minimise medication reviews and improve patient satisfaction
