Responsibility

  • support Practices to understand variation at a Practice level and seek to understand why variations exist
  • through the utilisation of data and liaison with the Practice Cancer Screening Champions, develop a thorough and ‘live’ picture of the Practice patient cohort
  • support practices to develop actions plans relating to cancer screening, identifying patients at risk and organising mobile/pop up clinics and informational events
  • all employees are required to observe the strictest confidence regarding any patient/client information that they may have access to, or accidentally gain knowledge of, in the course of their duties
  • the post holder shall act as a point of contact on the three national cancer screening programmes and will identify guidance and support to their Practices on all matters relating to cancer screening
  • liaise with PCN and Practice Managers to support Population Health Management
  • ensure timely reporting on areas of responsibility
