- carry out medicines optimisation tasks including effective medicine administration, supporting medication reviews and medicines reconciliation post-discharge and other transfers of care
- to provide information and answer medicines management queries from healthcare professionals and members of the public
- to undertake appropriate medicine optimisation activities that may include audit with GP practices as directed by the Clinical Director

- we have an Additional Roles team of 21 healthcare professionals and are looking for an individual with a passion to deliver excellent service in General Practice and in the local communities

- to provide technical assistance and administrative support, working with the Medicine Optimisation Pharmacists and clinicians in GP practices to implement and maintain high-quality, cost-effective changes in prescribing patterns and trends and reducing medicine related risk to patients

Requirements
- BTEC in Pharmaceutical Sciences/ QCF Level 3 in Pharmacy Services or equivalent
