- the PCN Patient Services Care Coordinator role is seen as a critical post to support the PCN Community Team, PCN Business and Operations Managers and the PCN Executive Team to help deliver and co-ordinate healthcare services to our patients
- to support the PCN staff by ensuring accurate and timely amendments to the patient booking platform including real time amendments where feasible for short notice absence. Alerting relevant stakeholders to help manage that absence
- to take minutes where required of PCN and MDT meetings and ensure that action points identified are recorded and updated
- coordinate multidisciplinary meetings across local care organisations including identifying patients in need of review and collating any information required to facilitate their review prior to the meeting
- assist patients and carers in managing their own needs, answering their queries and supporting them to address their needs
- working with all GP Practices within the Primary Care Network the post holder will take a lead on implementing a PCN strategy and plan to increase cancer screening rates across the three national screening programmes bowel, cervical and breast
- to deal with patient information in a confidential manner
- work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, be involved in co-ordination of care, and where required and as appropriate, refer people back to other health professionals within the PCN

- undertake any ad hoc reporting as requested by the PCN Manager or members of the PCN leadership team
