Responsibility
- to complete comprehensive assessments and use information accordingly
- to contribute to care planning and rehabilitation treatment planning
- to utilise outcome measures to monitor progress and optimise effectiveness of interventions
- to contribute to discharge planning, including liaison with community teams
- managing a caseload of patients who have complex presentations, using evidence based and client centred principles to assess, plan, implement and evaluate interventions
- to ensure all clinical records are maintained to the HCPC and RCSLT standards
- to work with patients and team members to identify goals and develop treatment plans in accordance with these goals; and to ensure regular review of goals during admission
- to contribute to regular reviews in Interdisciplinary Team Meetings and at planned review meetings; and to provide comprehensive and accurate reports as required
