Responsibility
to provide technical and administrative support to the Clinical Pharmacists and Clinicians

to work under the supervision of the Network Clinical Pharmacist to ensure the safe, accurate and timely supply of prescribed medication to patients
to support the primary care team with queries around medication from patients; including face to face and telephone support to patients, primary and community staff
nurse
Responsibility
the post holder will be part of a team delivering the service as per the agreed service protocol
the nurse/pharmacist will ensure patients receive optimal care and will determine what action is needed in accordance with the test result
there will be opportunities for case discussion plus trouble shooting with consultant support
it is important to us that you feel safe and supported in the role
the successful post holder will ideally be ARTP certified, however training can also be offered
this will include pre spirometry history/clinical assessment, spirometry +/- FeNO testing and post spirometry interpretation and reporting
the aim is to help identify undiagnosed cases, provide more accurate diagnosis for those with cardiorespiratory disease as well as optimising the clinical care and prescribing of patients with COPD, asthma and other diagnoses
we will work alongside the successful candidate to ensure they are sufficiently inducted and trained; there will be regular constructive supervision in place
care coordinator
Responsibility
the Care Coordinator will be a key part of our Primary Care Network
the role is patient facing and you will be expected to make proactive contact with residents
the focus and aim of this role is to support people with frailty needs within the Newham community.These will often be residents who have a range of physical, psychological and social support needs

residents will, in some cases, be known to existing community-based services across Newham but could be better coordinated across a multi-agency team
some people might not be known to services currently and could benefit from the holistic local offer approach you will be taking
the role will provide the glue that will bring together different professionals across Newham both members of the core Primary Care Networks but also wider services and voluntary and community sector as required
at the core of this approach will be person-centred discussions with a focus on developing trusting relationships and showing compassionate curiosity to ensure all involved understand what matters to people
