Responsibility

- document client preferences, care plans, and any changes in the client's condition
Requirements
- bachelor's degree in a related field or equivalent work experience
- proven experience in care coordination or a similar role within the healthcare industry
- excellent organizational and multitasking skills
- strong interpersonal and communication abilities
- knowledge of home care regulations and industry best practices
