Requirements
- A Diploma Level 3 in Health and Social Care or equivalent qualification or working towards
Responsibility
- ensure that all relevant information in respect of each member is communicated as appropriate to achieve delivery of person-centered approach
- champion the organization's vision and values, adopting an open and approachable management style supporting person-centered care
- we are passionately devoted to enhancing the lives of our members and their families
- actively support and implement effective Infection Prevention and Control measures
- complete all allocated one-to-one meetings and yearly performance reviews with staff in the department
- ensure appropriate staff allocations and completion of daily assignments
- co-ordinate the member assessment process, ensuring involvement of the resident in identifying needs and preferences, and integration of all information into the care plan
- regularly communicate to family members with regards to member needs and progress
