Responsibility
reporting any ill health of the members and make requests for GP/Professional visits where necessary

maintaining accurate records and ensuring that each member within the home has an individualised care plan
ensuring a clear and concise Handover Report is given to all relevant team members at the end of each shift
ensuring that all staff members contribute, to the best of their ability, to the efficient running of the home and the creation of an atmosphere conductive to the best interests of our members. Duties may include acting up for the Head of Wellness
administrating prescribed medicines and maintaining the appropriate records in accordance with the company’s procedure and NMC guidelines
establishing and maintaining good communication with members, and relatives
care coordinator
Requirements
A Diploma Level 3 in Health and Social Care or equivalent qualification or working towards
Responsibility
ensure that all relevant information in respect of each member is communicated as appropriate to achieve delivery of person-centered approach
champion the organization's vision and values, adopting an open and approachable management style supporting person-centered care

actively support and implement effective Infection Prevention and Control measures
complete all allocated one-to-one meetings and yearly performance reviews with staff in the department
ensure appropriate staff allocations and completion of daily assignments
co-ordinate the member assessment process, ensuring involvement of the resident in identifying needs and preferences, and integration of all information into the care plan
control of Substances Hazardous to Health, Electricity at Work, Lift & Lifting Equipment, Manual/Object Protective Equipment, Legionella/Water Treatment, Waste Management, Reporting of Injuries, Diseases & Dangerous Occurrences Regulations
