- arrange supplementary specialist assessments when necessary
- delivering a high quality clinical service to service users and their families
- delivering a high quality clinical service to patients and their families
- holding a caseload of service users and being responsible for offering a range of interventions to that individual
- to co-ordinate patients' care
- to promote and champion the rights of people with autism, working within the current service philosophy and values
- competently perform clinical skills related to the post
- working alongside
- managing a complex caseload and responsible for clinical and caseload supervision of other team members where identified
- supporting a service user through their treatment journey from beginning to end
- to formulate a timely discharge plan / Crisis care plan with individual/family/other services
- care coordination of an identified caseload with access to regular and robust supervision
- monday - Sunday working 5 shifts per week
- to identify patients who may benefit from alternative care pathways and to make referrals accordingly
- to provide a nursing service to prisoners taking account of cultural and spiritual beliefs/needs
- assess health needs at individual, practice and community level

- provide advice, support and guidance to colleagues and other professionals on issues relating to Home First
- delivering nursing interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties
- to be responsible for the appropriate risk assessment of service users, providing effective advice, guidance and decision making process
- understand how to use and maintain equipment safely and competently, following appropriate
- providing clinical interventions such as BBV's, wound care, vital signs, phlebotomy, ECG's, BBV investigations
- to undertake assessment of carer views and provide appropriate advice and support. To refer for a carer assessment if required
- assess patient's individual care needs, develop, implement and evaluate programmes of care
