Responsibility

  • the following are the core responsibilities of the Pharmacy Technician
  • this can include re-routing prescriptions to out of hours pharmacies for same day delivery
  • process monthly prescriptions for nursing/care homes, checking for changes and adherence/over-ordering issues
  • use practice and CCG templates to record near patient monitoring for high risk drugs
  • identify blood tests, recalls and reviews using established protocols and forward these to the appropriate clinician
  • there may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels
  • queries can include issues with interactions or ADRs, availability of stock, swallowing difficulties, cost-effectiveness, urgent requests or extra supplies
  • review hospital discharge notifications and outpatient hospital letters and reconcile medication against current repeats, flagging up and correcting discrepancies

Responsibility

  • greenwich PCN Alliance is seeking experienced Pharmacists
  • your experience will have been gained within primary care or another healthcare setting
  • the post holder will work as part of a multi-disciplinary team in a patient-facing role
  • ideally you will adequate skills to develop prescribing services by working across and with various stakeholder groups
  • the post holder will provide primary support to general practice staff with regards to prescription and medication queries
  • full and part time opportunities available
  • you will have experience of developing the need for strategic changes to prescribing practices and have a track record of succeeding in implementing these changes engaging with clinicians and patients internally and externally
