  • involve and enable the family and friends to continue caring for the patient and support them as appropriate To provide emotional support and communicate effectively with patients and their family/carers To work unsupervised in the patients home as required To take responsibility for-planning and allocating visits in the absence of more senior staff out of hours if required To role model the values of Farleigh Hospice and support others within the organisation to do so by recognising positive behaviours, challenging poor behaviour, encouraging and supporting change To work as a member of the multidisciplinary team to ensure the care plan for each patient is followed To support and work with the Clinical Nurse Specialists and Registered Nurses to develop and maintain effective partnership working with other FH Locality/Core teams To maintain collaborative working partnerships with external care agencies, GPs and community teams to facilitate best working practice and service provision To maintain accurate and timely healthcare records, where possible electronic based, liaising with team colleagues and in line with Farleigh Hospice policies and professional requirements To report accidents, incidents and complaints in area of responsibility as per procedure, participating in investigations as required To work to the processes for lone working, role boundaries and patient confidentiality To maintain personal and professional development including completion of all FH required mandatory training
