applicants must have held a Care Coordinator position in a domiciliary care agency for at least one year as this role requires the successful candidate to be able to start the actual work from their first day and hit the ground running
conducting telephone monitoring and service user visits and ensuring service quality

Requirements
at least one year’s experience as the Care Coordinator of a domiciliary care agency
familiarity with PeoplePlanner and Access Care Planning or other relevant software
