Requirements
health & Social Care Diploma Level 3 and above
previous experience in home care coordination or a similar role is highly desirable
strong organisational skills with keen attention to detail
● Experienced home carers looking to develop into a leadership role
excellent interpersonal and communication skills
● Strong communicators with an interest in mentoring and coordinating teams
● Organised individuals who can balance planning and hands-on support
ability to work independently and as part of a team
Responsibility
● Learn to schedule care visits and manage rotas
● Assist with client assessments, care plans, and family communication
● Support and supervise care staff, gaining leadership experience
● Maintain care records and understand compliance with CQC standards
scheduling: Coordinate and manage client schedules, ensuring continuity of care and timely home carer visits. Handle schedule changes promptly and efficiently
communication: Maintain open and effective communication with clients, caregivers, and their families. Address concerns and provide guidance as necessary
quality Assurance: Regularly monitor and evaluate the quality of care provided. Conduct periodic assessments and implement improvements as needed
documentation: Maintain accurate and up-to-date client records, care plans, and caregiver documentation. Ensure compliance with company policies and regulatory requirements
