Working hours
- full-time
Responsibility
- develop and implement individualised care plans for clients
- monitor and assess client progress and adjust care plans accordingly
- provide advice and support to clients and their families
- coordinate services between health care providers, social workers, and other professionals
- create the required rotas for care staff ensuring smooth operation of care services
- maintain accurate records of client information and progress
- provide crisis intervention services when needed
- develop strategies to assist clients in managing their own health care needs
Requirements
- the person has the appropriate combination of qualification, registration and experience as set out below (i.e
- a first level registered nurse on the appropriate part of the Nursing and Midwifery Council register; OR
- allied health professions registered with the Health Professions Council
- a professional social work qualification and registered on the appropriate part of the Northern Ireland Social Care Council register; OR
- either one of the professional qualifications under 1 combined with the experience under 2, or else the option set out at 3 combined with the experience under 4)
