- the North Community Nursing Team require a Clinical Practice Teacher

- to support the Clinical Team Leader in the delivery of the community nursing on a day to day basis
- to provide spontaneous and planned support, advice, teaching and instruction to the nursing staff, relatives and carers
- to promote understanding of the aims of community nursing team and to ensure a safe and consistent approach to patient care
- to be involved in implementation of any change to work organisation/service delivery
