Responsibility
- this is a varied, administration & customer facing information role providing dedicated support to a vulnerable cohort of patients
- the role includes completing Audits and reporting so a good level of computer literacy is required
- be able to work closely with Practice staff in a professional manner to nurture a spirit of collaborative team working
- work effectively alongside other teams and develop positive working relationships with colleagues and external stakeholders
- the post holder will be part of the wider Beacon Medical Group team and will require experience working with the public and vulnerable groups, having an awareness of the importance of safeguarding and data protection
- help create and get agreement on a long-term plan for the structure and processes to support the Multi-Disciplinary Team meetings and the ongoing patient case management
- promote a positive working environment
- the Care Coordinator role is seen as a critical and evolving post to support the delivery of personalised care to identified cohorts of patients within the PCN
