  • actively managing the rota on a daily basis
  • working in partnership with Carers and Customers to ensure person centred care schedules are provided
  • building and maintaining professional relationships with clients, families and other stakeholders
  • maintaining accurate and up-to-date communication records
  • ensuring staff files remain complaint
  • carrying out Electronic Care Monitoring audits and feeding back to Carers
  • managing and organising staff annual leave

Requirements

  • at least 1 year of Care Coordinator experience in a domiciliary care environment
  • NVQ Level 3 in Health & Social Care
  • full UK Driving licence
  • lead by example through working along side Carers when required
  • providing care: 1 year
  • supervisory: 1 year
